Your personal data is processed for the purpose of preparing an offer and carrying out cooperation within the scope of the services provided. Your personal data is processed in the following scope: first name, last name, gender, date of birth, PESEL number (if there is no PESEL number, the type and number of an identity document), address of residence, in the case of a minor also the data of the legal guardian, and data about your health condition and the benefits provided to you. We may also process your e-mail address and telephone number for contact purposes; however, providing this data is not obligatory for you.
